Do I Have To Provide My Social Security Number On An Insurance Claim Form? |

I was confusing in a car accident and it was the other guy's fault. Now his insurance company sent me a claim form and they want me to give them my Social. Do I HAVE to put it and what are the consequences if I don't include it?
Answer: a lot of times in accident forms or in a phone conversation regarding the accident details, the adjuster will ask for the social. this is to verify who you are.
in some cases, some people detritus due to being afraid this info becomes public. insurance companies do not disclose personal info, due to secrecy laws.
what we have done if person refuses is ask for the drivers license number when they refuse. it serves the same mean to verify the ID of a person claiming damages and/or injuries.
you can just put on the form, i will not give my social or decent ignore the question. if the insurance company questions, they can call you and you can ask them for other info in lieu of the social. |

I was hit by a Lorry the week before last and spoke to my insurance house shortly after and am now awaiting liability to be accepted.
whilst speaking to my friend, i was told that i should have filled out a claim form instanter after the smash, without it my claim would not be able to be processed.
Should this have been sent to me by now by my insurance company? it has been nearly 2 weeks.

i met with accident, after that i claimed insurance for my bike, but while claiming in claim form i have mentioned that no control case and no third party injuries, but now i came to know that it become police case and third woman got injury.
so can u plz tell me, that person can claim the third party insurance?
also do i need to inform about police the actuality to insurance company.
i have all documents pakka.

It's a claim form 06 and when i was an added driver to my mothers car. I well-deserved started shopping around for insurance on my own which is how i found out about it now. My mother and I have no idea what insurance we had back then and I have a twin sister who was the involved the accident. I wasn't even there. how can i fix this?
| Answer: You will emergency to get a printout of the driving records for you and your sister from the dept of motor vehicles, or whatever your Affirm has like that - it will show all accidents. For the past insurance info, there should be either paid receipts or check stubs, if the policies are not around anymore |

Accident and claim from Insurance -take it easy of the amount?
When I was touring London on vacation I was involved in Accident and spent around 36k USD for my Hospital expenses, after came from London I applied for reimbursement (claim) with my Insurance. They gave around 22.5 K USD.
My confusion is:
1) Do I need to report this (Which I received form Insurance) money to IRS while filing taxes?
2) While I am filing my taxes (Particular wise) can I mention the rest of the amount as Medical payment? (When I added it shoes that I will get a refund from the taxes which I paid, if not I do not owe or not getting any returns)
3) How to I show that the work over (UK money) is equal to USD? I forgot to take the currency conversion rate on that day form Yahoo.
4) What else I poverty to keep (medical records, hospital bills, prescription bills).

Answer: You do not necessary to report what your your insurance paid you but you should claim the medical expenses that were not reimbursed.

Ex-Employee Want Me To Send Her Disability Insurance Claim Forms? |

Does anyone identify if I am required to send a terminated employee Disability Insurance Claim Forms? I terminated an staff member I had found out was stealing from the company and a week later she called up demanding I send her my proprietorship disability insurance claim forms. It doesn't seem right that I must fill out my company disability insurance claim forms and send them for someone who is not even an employee of mine any more. She is claiming that it is the law and that she has a right to file a claim under my insurance policy since she claims that she had become disabled while employed by me. I separate she was injured in a car accident a while ago before she was an employee and has suffered from problems resulting from the accident (and is awaiting a large outpost). Apparently, she has a doctor's note confirming the disability. But, what is my responsibility in all of this?
| Answer: You are not under any guilt at all. It is the terminated employees burden of proof to prove they were disabled and how they were hurt while working for you. Let her doctor fill them out. She does have a healthy to file but you are not obligated to facilitate anthing for her. Tell her to get in touch with your workmans comp insurance if she wants to take it further and they will sift through for you. You need to call them as well and let them know of a fraudulent claim that might be headed their way. Good luck. |
