How To Speed Up Insurance On Disability Claim? |
May 19, 2007 by donnacmmngs | Posted in Insurance
we have supplemental disability and we filed in strut as husband had septic shock...now it is may and they say they are still waiting on records from hospital(military)...how can i race it up?as im gonna be evicted soon
Since their condonation seems to be lack of substantive records to authorize your claim, ask them what records they are waiting for and from whom. Then courteously call the slack agency/hospital involved, explain that their delay is causing serious hardship, and inquire if there is any way you can expeditiousness these missing records to the insurer.

Ex-Employee Want Me To Send Her Disability Insurance Claim Forms? |
May 12, 2008 by Diva_62 | Posted in Small Business
Does anyone recall if I am required to send a terminated employee Disability Insurance Claim Forms? I terminated an employee I had found out was thievery from the company and a week later she called up demanding I send her my company disability insurance claim forms. It doesn't seem at once that I must fill out my company disability insurance claim forms and mail them for someone who is not even an employee of mine any more. She is claiming that it is the law and that she has a right to file a claim under my insurance way since she claims that she had become disabled while employed by me. I know she was injured in a car accident a while ago before she was an employee and has suffered from problems resulting from the mischance (and is awaiting a large settlement). Apparently, she has a doctor's note confirming the disability. But, what is my trust in all of this?
You are not under any culpability at all. It is the terminated employees burden of proof to prove they were disabled and how they were hurt while working for you. Let her doctor fill them out. She does have a above-board to file but you are not obligated to facilitate anthing for her. Tell her to get in touch with your workmans comp insurance if she wants to take it further and they will study for you. You need to call them as well and let them know of a fraudulent claim that might be headed their way. Good luck.

I Filed My California Long Term Disability Insurance Claim 40 Days Too Late And It Was Denied. Can You Help? |
Dec 12, 2008 by harsuwind | Posted in Insurance
I am covered by a gruff term disability policy by my employer. Prior to my disability, I asked my HR department what I needed to do. I was told that my knee-high to a grasshopper term policy would kick in automatically and all I would need to do was fill out those forms once they were sent to me. I filled out the forms and received confirmation that my dwarfish term insurance had been activated. About 1 month after my disability, I received a reduced paycheck. When I inquired at industry, I was told that I should have filed for long term disability along with the short term disability. I have since filed for desire term disability but it was denied because I submitted my claim more than 40 days after the first day of disability. I know I can appeal. Has anyone else gone through this and was your be attractive to successful? Did you have to do anything out of the ordinary to win your appeal?
Advising me to check with the labor board or my HR trust in isn't the help I am looking for. What I am looking for - Have you ever had to appeal a denied disability claim and did you learn something in the process that would plagiarize make the process go smoothly for someone in my situation. If I were answering your questions, I would put some real anticipation behind my answers!
You have the right right to appeal the claim decision, but keep this in mind - the insurance carrier follows the contract that they have with your proprietor. If your employer wanted a late filing clause, and the insurance carrier is just following the go down with, then the appeal isn't going to do much good. If I were you, I would get a copy of your disability policy so you can verify that this late filing clause indeed exists, file an appeal with the insurance carrier, and file an appeal/grievance with your stage's insurance board. I think that these three actions are your best bet for possibly overturning the decision.
Company luck!

Is W-2 Box14 Entry For CASDI (California State Disability Insurance)a Valid Claim On Form1040 Sched A Line5? |
Mar 18, 2007 by jr | Posted in United States
Instructions for Offerings5 say to include "Mandatory contributions made to California, New Jersey, or New York Nonoccupational Disability Perks Fund, Rhode Island Temporary Disability Benefit Fund, or Washington Articulate Supplemental Workmen's Compensation Fund" which seems vague at best. I have scoured the web for intelligence addressing this question. CASDI is certainly mandatory.
If you are itemizing your deductions you can take in the CASDI as State Income tax paid on line 5 of the Schedule A

I Need A Disability Insurance Attorney? |
Nov 10, 2009 by Fletcher O | Posted in Insurance
Six months ago I got into a bad car mishap (not my fault) and I haven't been able to drive or got to work or even perform any of my duties. I miss a lawyer who can help me get disability insurance claims from my employer or else I'm going to have to move in with my parents.
Lawyers are admitted to in real life inexperienced law in each state. You need a lawyer in your state, so you should list that.
Having said that, does your chief PROVIDE you with disability coverage? If so, long term, or short term? And are you MEDICALLY incapacitated, as in, a doctor has declared you unable to work?
LOTS of questions here, but if you are really non-functioning, AND, there IS coverage (many employers do NOT provide any kind of disability coverage!), any local bencher should be able to do this for you.

Panic/Anxiety & Depressive Disorder Disability Claim With Private Insurance Company Denied. Advice Please? |
Nov 27, 2007 by Mocha8264 | Posted in Mental Health
I am tribulation from a panic/anxiety disorder which has brought about severe depression. I have been unable to think, eat, sleep, work, or socialize with family or friends. I am seeing a therapist & we are working my way out from this nefarious hole.
My question centers around the STD disability claim that I filed w/my employer's insurance company. They denied my claim citing be without of comprehensive doctor's notes to support the diagnosis; and they also said that I've been able to go to boarding-school without missing any classes (which is not true...I've had to fight to get to class and class is only 1 hour extensive, 3 nights a week...vs. work which is 8 hrs, 5 days).
So, my question is....has anyone else been denied from their restrictive insurance carrier for STD due to anxiety &/or depression? And if yes, what would you suggest I do from here to get them to reopen my claim & approve it?
NOTE: Anything you can victual regarding my claim would be helpful. I only ask that everyone be kind.
As satisfactory as it is to have insurance, sometimes insurance companies really suck.
My suggestion would be to have your therapist contact them (or whoever deals with insurance tripe in your doctor's office) and see if something can be worked out. Most likely your therapist is submitting notes that are thumbnail and minimal (probably for confidentiality reasons for your sake) and that's why the insurance company is not covering your distress.
I would also say that you need to contact your insurance company and talk to them as well. You will be the best person to lobby for your own control because you can best convey what's going with you. Insurance companies can be stubborn though, so don't be surprised if they're not willing to be obedient with you.
I completely understand your reasons for being frustrated with your insurance company. First of all, I don't believe it's any insurance company's issue as to what I talk about in my therapy sessions (I've run into this problem before too - insurance was wanting my doctor's notes, a group history on me, results of psychological testing, etc. Fortunately, my therapist's office was masterly to sort out everything and I'm able to get coverage). Second of all, I also don't believe that insurance companies should demand what kind of care they will cover and how long they will cover it for. When they become mental health specialists, then they can requirement that kind of stuff :) (Sorry, my own personal rant about insurance coverage :)) I occupation on an adolescent psychiatric unit and this is something the case managers are constantly dealing with when it comes to insurance companies - getting them to grant to coverage and getting them to understand that our patients really do need inpatient psychiatric protection.
I don't feel as though I've been much help to you...all I can say is that I definitely understand where you're coming from and the frustration that comes with dealing with these companies. I get the drift the depression and anxiety as well, as I'm being treated for both right now. When a person is feeling that bad and is in desperate deprivation of treatment, it can be scary that insurance could stand in the way of your care. Advocate for yourself, ask your therapist to talk to the insurance Pty as well. Hang in there and best of luck to you :)
