I have received the 1500 Health insurance claim form from a rite provider. I want to ask some questions.
What is 1500 Health Insurance claim form?
When I receive this form, what will I do next?
Answer: The CMS 1500 (formerly HCFA 1500) is a form adapted to for medical billing. Its what all professional providers use to bill medical claims. (Institutional providers - ex. hospitals - use the UB-04.)
If your provider gave you a 1500, then I'm assuming that you're obliged to submit it to your health insurance company for reimbursement. That's the only reason you'd ever need one of those forms - to get reimbursement from a health insurance companions.
Did your provider complete the form for you, or do you need to fill it out yourself? If its already filled out for you, then just make a copy for your records and despatch the form to your insurance company's claims address. |

Hi, I'm wondering what to fill in the passive information section? Is that me myself? Or someone else who's under my insurance plan? There's a table to fill in, and each has to indicate the relationship with the plan colleague (me), so I'm wondering if this information is for 3rd party other than me myself? Thanks.
Answer: The persistent information section will be the details of the patient, if you are the patient then it is you, if it is someone else covered by your policy then you co-sign their details and their relationship to you.
Where it is you who is the patient enter "Plan Member" in the box asking about relationship to down member, hope that helps you. |

Answer: It may be a carry on code. There are two kinds of codes needed to file a health insurance claim, CPT code and ICD-9 codes. CPT is the gain code and ICD-9 is the diagnostic code. Go to this site, enter your state and the code and see what comes up.

I'm currently attending a taste about Medical Office Procedures, but my teacher sucks. She gave us 5 weeks to write 'finis' to all the assignments and she doesn't help us unless we're in class. I have a total of 87 assignments and I'm currently stuck on the health insurance forms. The precinct to the left of the form, 1a. - 13a, has me confused.

Do I fill the form before or after successful to the actual doctor?
Answer: It depends on the comrades, the policy, and how the policy is set up. If the policy is a network policy it also depends on the doctor you see. If it is a network behaviour the policy may or may not pay ANY claims if the doctor is not in the network. You'll need to contact the insurance company because nobody here will be aware.
Generally, with most policies the doctor will file the claim with the insurance, not you. |

I filed a claim with my health insurance transporter (the provider is out of network and does not file for me).
I sent the claim USPS Certified Dispatch, including all necessary claim forms and an explanation of the claim.
How long should I give the insurance company to respond before following up? Is 15 subject days enough, or should I give them more time?
Answer: Legally, they have at least 30 days. (Some states brook 45.)
But realistically, most insurers should at least have the claim entered into the system and pending within 2 weeks.
P.S. I know you didn't ask about this, but proper to save you another question later on...odds are you won't get the full amount you're expecting back.
Your insurance company is going to answer based on a UCR (Usual and Customary Rate), not the provider's actual billed debit. For example, if you're expecting 60% back, they are going to reimburse you 60% of the reasonable and accustomed rate for the service. Not 60% of the amount you actually paid. |

Mom who is 81 and has circumscribed medical knowledge or verbiage, took out a full policy from a company in Canada, in behest to cover herself 100% for a catastrophic health event during her 7 week stay in USA. She misunderstood the huddle "puffer" and stated that she does not use one in her answers on questionnaire. The decorous word "inhaler" was not used on form, only the word "puffer" Although the use of an inhaler, which she almost never needs anyway, and she did not even bring on her trip, is used on occasion for a cough she sometimes has. The use of an inhaler has nothing at all to do with her catastrophic at any rate that has her in a coma and on life support in the USA. (an enteric aortic fistula) She has been penalized, and her claim for medical expenses denied for not answering correctly and saying that she uses a "puffer" They say that the game plan is void even though they accepted this 81 year old widow's thousand dollars to insure herself for 7 weeks. It is not known if my mom will come through

Answer: Well, I can't selected specifically in terms of medical offices because I am a dentist. So I will tell you how it's done at my office. The tools we use to congregate insurance billing information are: 1. We utilize a book of billable dental system codes so that we can file insurance claims properly. 2. We subscribe to a special software that keeps footprints of insurance benefit plans by employer. 3. Once we have basic insurance information we request clear-cut benefit coverage of the policy by either calling the insurance company directly, faxing them, or online.
I do not negotiation with medicare so you will need to research that on your own. As far as patient billing goes our policy is uninvolved. Payment is due at time of service. If patients can't afford to pay their portion we accept mastercard and visa. I also proffer 3rd party financing with a company called Care Credit. If we have to send a bill the staunch has 30 days to pay the balance and if it is not paid within 90 days a late fee is assessed and their wrongdoer accounts are reported directly to all 3 credit bureaus.
