How Many People Have Been Hurt Fighting Against Health Care Reform And Had Their Health Insurance Claim Denied? |
Sep 04, 2009 by Count Acumen | Posted in Politics
Wouldn't that be very apart for someone to literally get physically hurt fighting for the health insurance companies only to find out that when they filed their claim to the insurance company their claim would be denied ? It's not as far fetched as you might create.
Well at least with a sequestered company, you can fire them if they don't deliver on their promises. I was injured fighting for my country. I'm a 100% marines-connected disabled veteran. My country promised that they would take care of my health if I risked my lifestyle for it. Now that I'm disabled I find that those promises are completely worthless. The VA delays, deflects, and denies treatment to non-functioning veterans. I have faced blanket denials, long delays, refusal to stock up medication, treatment, or tests. Refusal to pay any and all health care costs in the private sector, which they promised would be covered. How much more lowering is it to give everything you have to your country, and then find that they have no intention of keeping the promises made? No, its not far-fetched at all. I have "free" health attention. It is worse than having nothing. Unkept promises do more damage than promising nothing at all. At least when people distinguish they are on their own they can make arrangements for their care. To be promised care that is never delivered keeps people clinging to expect until they die from neglect from those that they trusted.

What Is 1500 Health Insurance Claim Form? |
Dec 27, 2008 by ntthang_vn | Posted in Insurance
I have received the 1500 Health insurance claim elevate from a service provider. I want to ask some questions.
What is 1500 Health Insurance claim form?
When I receive this materialize, what will I do next?
The CMS 1500 (formerly HCFA 1500) is a cut used for medical billing. Its what all professional providers use to bill medical claims. (Institutional providers - ex. hospitals - use the UB-04.)
If your provider gave you a 1500, then I'm assuming that you're imagined to submit it to your health insurance company for reimbursement. That's the only reason you'd ever need one of those forms - to get reimbursement from a health insurance establishment.
Did your provider complete the form for you, or do you need to fill it out yourself? If its already filled out for you, then just make a replica for your records and mail the form to your insurance company's claims address.

Help With Health Insurance Claim Form: What To Fill In The Patient Information? |
Jan 26, 2008 by tictac | Posted in Insurance
Hi, I'm wondering what to fill in the acquiescent information section? Is that me myself? Or someone else who's under my insurance plan? There's a table to fill in, and each has to indicate the relationship with the plan associate (me), so I'm wondering if this information is for 3rd party other than me myself? Thanks.
The acquiescent information section will be the details of the patient, if you are the patient then it is you, if it is someone else covered by your policy then you puncture their details and their relationship to you.
Where it is you who is the patient enter "Plan Member" in the box asking about relationship to representation member, hope that helps you.

Can You Appeal Health Insurance Claim Denial? |
Mar 11, 2008 by Christy S | Posted in Insurance
I am frustrating to have a breat reduction and the my health insurance company had denied it. What can I do?
I do have back problems: neck problems, and always have onerous muscles. The Dr. said in the initial visit that it would probably be denied: He was planinning on an sue with a panel of Phycisians.
Christy, bust reduction is considered a cosmetic surgery and such a claim is not allowed as the terms and conditions have specified apparently. However, if the doctor's report can clearly prove that it is needed on medical reasons; then your claim should be approved. Do croak review with your insurance agent for more details.

How Can I Effectively Appeal A Denied Health Insurance Claim? |
Jul 24, 2008 by ChyTay | Posted in Insurance
I have a series of claims ...for chiropractic treatment that has been denied....how can I get these appealed successfully?
The treatment was decompression group therapy...and the insurance came back and said that it's not a covered therapy.
The form where I live mandates that chiropractors are a covered provider. Lot's of states don't have that mandate. Before you go through lots of affliction appealing the denial of claims, make sure that your policy covers such treatment.
The first journeying is with the insurance company. Step two is with your states insurance commissioners office.
Good luck

How Do I Fight My Denied Health Insurance Claim? |
Sep 15, 2009 by Nicholas C | Posted in Insurance
While on vacation in AZ, I visited a CVS instant clinic for my sore throat. The cost was $107 but Blue Shield wouldn't coat it, claiming in a letter that by visiting CVS, i had literally designated MYSELF as my own doctor instead of my conventional primary care physician! The letter even stated "M.D." next to my name as the main care physician that I visited at CVS!! How do I fight this? Who can I talk to?
Perhaps your provider is muddled? They may think you 'self-medicated' with OTC treatments purchased at CVS, rather than seeing a doctor at the clinic. I would call and talk to someone there, and untangle justify the situation. First, you can visit the Minute clinic site, and check that your provider covers their services:

The Shake Clinic site recommends you check with your insurance provider before treatment:

What Does The Number 60054 Mean On The Aetna Health Insurance Claim Forms? |
Feb 03, 2008 by + _JOSEPH_ + | Posted in Insurance
I've seen that numbers on those forms but the performers doesn't want it, i'd like to know why
It may be a move along code. There are two kinds of codes needed to file a health insurance claim, CPT code and ICD-9 codes. CPT is the approach code and ICD-9 is the diagnostic code. Go to this site, enter your state and the code and see what comes up.
my guess is that your insurance doesn't disguise that procedure.

Straw men and opportunists threaten health-care reform
02.12.09
Straw men and opportunists warn health-care reform It’s always easier to do nothing. And that’s what Republican opponents of health-care reform are banking on. In any proposed servicing of something as complicated as health care in America there’s a thing or two that will stick in just about anyone’s craw. It amounts to socialized remedy. It will use tax dollars to fund abortions. It will undercut Medicare. It will create a giant observations base and people will be assigned ID cards. It will raise taxes. It will saddle tomorrow's generations with higher costs.
